To ensure that your first appointment is successful, we would like to take a minute to go over the process and all details you should need. We want this to be a comfortable, enjoyable visit!
At your first appointment at Clintonville General Dentistry, you should show up 10-15 minutes early to go over paperwork and to become oriented into the practice. You will be greeted and get checked in with Natalie, and then will meet with the hygienist. After a thorough dental cleaning, you will have the opportunity to consult with Dr. Gardner and discuss your smile goals.
Be sure to bring your driver’s license and insurance card. If you would like, please print and fill out the patient forms on this page.
If you are coming into the office with pain, please let us know! We will ensure that you see the doctor right away.
Prior to your first appointment, you will receive a confirmation phone call from Natalie. This is for your convenience and ease in scheduling. If you need to cancel your appointment for any reason, please call at least 48 hours prior to your visit. If you cancel without any notice three times, we reserve the right to refer you to another dentist.
